CMS GUIDE Model

In 2023, CMS announced the GUIDE model, or “Guiding an Improved Dementia Experience”. This voluntary model aims to improve the quality of life for dementia patients and their informal and family caregivers with an approach to care delivery that prioritizes care coordination and caregiver support.

GUIDE Model Requirements

Care Coordination & Management

Interdisciplinary team creates a person-centered care plan involving continuous monitoring and care.

Caregiver Support & Education

A training, education, and support group for caregivers. Expert Care Navigators facilitate access to clinical and non-clinical resources.

Respite Services

Caregivers receive respite services costing up to $2,500 a annually.

Navigation

A dedicated expert helps facilitate communication, appointments, & medication with the clinical care team. Referrals to community services.

24/7 Support

Beneficiaries and caregivers must have access to a member of their care team or navigator using a 24/7 helpline.

Training

On-demand access to comprehensive educational classes, content, and support groups on a wide array of topics related to dementia caregiving.

Medication Management

The clinical team regularly reviews prescribed medication as needed, while care navigators provide tips to successfully adhere to medication regimens.
