ianacare Announces Patient and Caregiver Engagement Solution for CMS GUIDE Model

Boston, MA – July 15, 2024 – ianacare, a leading provider of tech-enabled, comprehensive support services for patients and their family caregivers, announced the launch of its digital front door solution designed to engage families and meet the caregiver support requirements of the CMS GUIDE model. Built leveraging ianacare’s experience serving over 45,000 patients and their family caregivers, this comprehensive caregiver engagement platform is the first of its kind to offer an all-encompassing solution to meet care delivery requirements for both established and new track GUIDE entities.

The ianacare platform provides an easy-to-access engagement point for caregivers, enabling them to:

  • Receive caregiver training and education
  • Access referral and coordination services for community support
  • Access, navigate, and manage respite care
  • More easily manage medication adherence
  • Benefit from ongoing monitoring and support from licensed Caregiver Navigators
  • Utilize a 24/7 support phone line for immediate assistance

The solution is available in several configurable tiers, allowing GUIDE participants to tailor the solution to their specific needs, for both new and established track participants.

“The GUIDE model provides an innovative level of support to families and caregivers of patients with dementia, but GUIDE participants need a proven platform to engage them,” said Steven Lee, Co-founder and CEO of ianacare. “By providing a robust, flexible platform proven to deliver many of the care delivery requirements, we empower GUIDE entities to effectively support caregivers and meet program requirements, ultimately improving outcomes for caregivers and their loved ones.”

“We are honored to be able to turn our deep experience serving patients and their caregivers to provide much-needed relief for GUIDE beneficiaries,” said Jessica Kim, Co-founder and Executive Chairwoman of ianacare.

ianacare has been providing these categories of support for years and has proven results that show deep caregiver engagement and trust. Since 2018, the platform has served over 45,000 caregivers and patients, and trained over 6,500 caregivers, with an average of 21 touchpoints per care episode.
