Report | GUIDE Model Participant Survey 2025: Data & Insights

In continuation of last year’s survey, we collaborated with Dr. Mia Yang from Wake Forest University Health Sciences to conduct a follow-up survey of GUIDE model participants, which includes both Established and New track organizations.

As participants delve deeper into the program, we wanted to understand the shifts in program implementation and beneficiary care approaches since last year. This comprehensive report summarizes our findings, offering valuable insights into how diverse organizations are navigating the GUIDE model.

Key Takeaways:
  • How various organizations are effectively implementing the GUIDE model.

  • The unique challenges participants are encountering and the innovative solutions they’re employing.

  • Best practices and shared opportunities for growth and collaboration within the GUIDE community.
