GUIDE and Respite: Bridging the Gap in Dementia Caregiver Support

Caregivers are burning out, despite growing awareness and programs like the GUIDE model, due to critical gaps in respite care access; this article explores how innovative solutions, including community support and technology, are essential to truly support those providing care.

At ianacare, we have long recognized what healthcare is only now beginning to prioritize—family caregivers need meaningful support to sustain their role and ensure better outcomes for both themselves and their loved ones.

Since our founding in 2018, we’ve seen firsthand how respite services are among the most critical ways to offer this support. Our data, combined with countless real-life accounts, confirm a simple yet powerful truth: giving caregivers a break not only makes them happier, healthier, and more effective in their role – but, it’s critical to prevent burnout which impacts the quality and sustainability of this care. As more organizations incorporate the CMS GUIDE model to better serve those caring for loved ones with dementia, respite care must be at the center of our efforts.

“For ten consecutive years, I was the primary caregiver for both parents while also raising my three young children and working full time. The chronic stress and non-stop 24/7 nature of caregiving had a deeply negative impact on my own mental and physical health. I did some research on getting temporary help, but the options were unrealistic and unaffordable,” says Jessica Kim, co-founder and Executive Chairwoman of ianacare.

Yet, as awareness grows, efforts to address the true scale of the issue remain constrained, leaving a vast gap between recognition and action. Three major limitations stand in the way of providing meaningful respite:

Barriers to Respite Access

  • Eligibility Requirements: Respite services are only available to caregivers of beneficiaries in the moderate or high-complexity dyad tiers, leaving out many caregivers who would benefit from similar relief.

  • Funding Constraints: The $2,500 annual allocation is a pivotal step forward, but we need to build upon this foundation. It covers only a few hours of respite per week or a handful of full days per year—far from what’s needed to fully meet caregiver needs in order to prevent burnout.

  • Provider Availability: CMS reimbursement rates for respite providers are challenging in many markets, as they can be lower than market rates, making it difficult to attract qualified providers in numbers that guarantee coverage, especially in more rural areas of the country, where care is already a challenge to find.

For these reasons, when we surveyed GUIDE model participants, respite care was the requirement they felt least equipped to fulfill and the one they were most likely to seek external vendor partnerships to fulfill. The entire care delivery ecosystem is struggling to meet the demand for respite, and it’s time we got creative about meeting caregiver needs while also acknowledging these challenging factors. At ianacare, innovation is at the core of our approach, and we have spent years finding new, scaleable, and reliable avenues for caregivers to receive the support they need – including respite care.

“Respite care needs to be a core part of how we support family caregivers. Temporary relief is not a one-time instance. It needs to be part of the ongoing care plan specifically designed for the caregiver’s situation that evolves with the changing needs. To complement GUIDE’s generous respite care funds, ianacare is taking a holistic approach that brings together the fragmented system into a cohesive solution. I’m so proud that we’re able to address this difficult problem through our innovative tech, strong network, and personal navigation,” says Jessica Kim.

Respite Through Community

For the past eight years, ianacare’s platform has transformed the way caregivers access respite by facilitating both formal and informal support options. 📣 In 2023 alone, our platform helped organize nearly 1,200 respite breaks, with 50% provided by non-familial supporters such as friends, neighbors, and coworkers. This highlights a crucial reality: caregivers’ personal networks can play a major role in bridging the gaps left by paid respite services. With the right structure and technology, we can unlock this invaluable resource.

Respite Through Local Resources

Many caregivers using ianacare have also discovered community-based respite options through our extensive network of local resources. In 2023, some of the most searched terms in our platform included “adult daycare” and “relief for caregivers.” These searches reflect a growing need for alternative respite options that go beyond formal programs. Our technology connects caregivers with these resources through a proprietary database, empowering them to access support in ways that fit their unique situation beyond a basic google search.

Respite Through Caregiver Navigators

Our Caregiver Navigators work closely with caregivers to assess their unique needs and stress levels, providing tailored recommendations on how to best utilize available respite care resources. By aligning solutions with caregivers’ lifestyles and their loved ones’ specific needs, we ensure respite is not only accessible but also truly effective.

An Appeal for Impact

Respite care is not a luxury—it is a necessity. The GUIDE model provides a critical foundation, but it cannot stand alone. We must push beyond traditional boundaries to build a future where every caregiver has access to the breaks they need. At ianacare, we are leading this charge, leveraging community networks, local resources, and innovative technology to ensure that respite is available, sustainable, and impactful. Now is the time for healthcare systems, employers, and policymakers to join us in this mission. Together, we can redefine what it means to support caregivers and create a world where no one has to navigate caregiving alone.
